Most people regain weight after stopping a GLP-1. Here is what the research shows, what is still unknown, and how to build an off-ramp before you need it.
Quick answer
Most people regain weight after stopping a GLP-1 medication. In one major trial, participants regained about two-thirds of what they had lost within a year. No approach has been proven to prevent this, but habits built while you are still on the medication, especially strength training, protein, and a structured eating plan, give you the best footing. Do not stop a prescribed medication without talking to your prescriber first.

People stop GLP-1 medications for many reasons. Cost, insurance changes, side effects, reaching a goal, or simply not wanting to take a medication long term. Whatever the reason, the same question follows: what happens to the weight?

What happens when you stop

GLP-1 medications quiet your appetite and slow digestion. When you stop, those effects fade and hunger returns. Stanford Medicine summarizes the best-known data on this: in an extension of the STEP 1 trial, people who stopped semaglutide had regained two-thirds of their lost weight a year later.

This is not a willpower failure. Obesity specialists compare it to blood pressure medication. When the treatment stops, the condition it was managing tends to come back.

There is one more finding worth knowing. A 2025 study covered in The Pharmaceutical Journal found that people regained weight faster after stopping medication than people who had lost weight through behavioral programs. One reasonable reading is that medication can take weight off without building the habits that help hold it off.

What we know and what we don't

It is worth being honest here. A 2025 joint advisory from four obesity and nutrition organizations lists nutrition after stopping a GLP-1 as an area that still needs more research. Anyone promising a guaranteed way to keep the weight off is ahead of the evidence. What follows is what experts reasonably recommend, not a guarantee.

Build the off-ramp before you need it

  • Plan the exit with your prescriber. Whether to taper, stay on a lower dose, or stop is a medical decision, especially if you take the medication for diabetes.
  • Protect your muscle now. Muscle supports your strength and your metabolism. Strength training at least three times a week and protein first at meals are the two levers. See our guide on what to eat on a GLP-1.
  • Practice your eating structure while your appetite is quiet. Regular meals, enough protein and fiber, and fewer refined carbohydrates are easier habits to learn when you are not fighting hunger.
  • Know your numbers. A body composition scan before you stop gives you a baseline. Follow-up scans can show fat returning before your clothes do. More on that in fat or muscle: how to tell the difference.
  • Look after sleep and stress. The advisory includes sleep, stress, and physical activity as part of the plan, not extras.

How First Line Therapy fits

First Line Therapy is our 12-week lifestyle medicine program. It includes an initial health assessment, a personalized nutrition plan, regular consultations with your provider, six body composition scans, and a transition plan for the long term.

It works well as the structure for your final months on a GLP-1 or your first months off one. It is also an option if you would rather start with lifestyle changes before considering medication, or if a GLP-1 is not right for you.

We won't tell you it replaces the medication. What it gives you is a plan, accountability, and measurements, so you are not finding out how things are going by guessing.
