If you have started searching for Ozempic back pain or joint pain on a GLP-1, you are probably trying to work out whether your medication is behind it and who you should call. The answer depends mostly on where the pain sits and what comes with it, so here is what the FDA labels say and how to choose your first appointment.
Is back or joint pain on a GLP-1 a listed side effect?
Not as a common one. We checked the current labels for Wegovy (revised February 2026) and Zepbound (revised April 2026), and neither back pain nor arthralgia, the medical word for joint pain, appears in their main adult side effect tables. Wegovy and Ozempic share the same active ingredient, semaglutide.
Those tables only show reactions reported in at least 2% of people and more often than with placebo, so a missing entry does not prove a symptom never happens.
Why might your back or joints hurt during weight loss?
Losing weight quickly can change how you stand and walk, and it can change how much muscle is supporting your back and hips. Both labels say these medications lower weight with greater fat loss than lean mass loss, which also means some lean mass goes with it, and our article on losing fat or muscle on a GLP-1 explains why that matters.
Both are sensible things to check during an exam, but we have not seen solid research testing them in people on GLP-1s. Cleveland Clinic names strains and sprains as the most common cause of back pain overall. Our colleagues at Aim High Health in Colorado have a helpful overview of back pain treatment.
In Wegovy's cardiovascular trial, hip and pelvis fractures were reported more often than with placebo in women (1% vs. 0.2%) and in people 75 and older (2.4% vs. 0.6%). If you have hip or pelvic pain after a fall, get it checked instead of treating it as a muscle strain.
When could back pain be a sign of something else?
Some back pain starts somewhere else. Cleveland Clinic explains that pain can be referred to the back from organs including the kidneys, gallbladder and pancreas. Both labels warn about acute pancreatitis, describing persistent or severe abdominal pain "sometimes radiating to the back," and both warn about gallbladder disease. They also warn about acute kidney injury, mostly in people who became dehydrated from nausea, vomiting or diarrhea.
If pancreatitis is suspected, the labels say the medication should be stopped, so your prescriber needs to hear about that kind of pain quickly. Please leave any change to your dose to them.
Can a GLP-1 ease knee osteoarthritis pain?
For some people, it may. The STEP 9 trial, published in the New England Journal of Medicine, followed 407 adults with obesity and moderate knee osteoarthritis for 68 weeks. Everyone had diet and activity counseling, plus weekly semaglutide 2.4 mg or a placebo.
Weight fell 13.7% with semaglutide and 3.2% with placebo.
That result is encouraging, with limits. The study looked at knees in people with obesity, so it tells us nothing about back pain, and it can't show exactly why the pain eased. Novo Nordisk, which makes semaglutide, funded the trial, and knee osteoarthritis is not a listed use on either label.
Who should you see first?
- Your prescriber or primary care provider: Start here if you have any warning sign above, if the pain began alongside stomach symptoms, or if you have new joint pain without an obvious reason. Primary care is also a good place to go if you can't reach your prescriber.
- A chiropractor or physical therapist: This is a good fit once serious causes seem unlikely and your pain changes with movement, sitting, standing or lifting. Cleveland Clinic includes physical therapy and chiropractic adjustment among its back pain treatments and notes that gentle movement often helps more than bed rest.
- Not sure which one? Call a doctor about back pain that hasn't improved after a week, as Cleveland Clinic suggests. Our guide to lower back pain in Macomb County can help you decide.
Where does Total Health Systems fit in?
Decisions about your medication belong to your prescriber, and we are glad to work alongside that care. If your pain appears mechanical, our chiropractors and physical therapists can assess how your back, hips and joints are moving, and our goal is to help you stay active and comfortable. If something points away from muscles and joints, our goal is to get you to the right provider quickly.
If you want to see how your weight loss is affecting your muscle, a $60 InBody scan takes 45 seconds and reports skeletal muscle mass and body fat percentage.
This article is general education and is not medical advice. Talk with your prescriber or primary care provider about new or worsening symptoms, and do not change your GLP-1 medication without their guidance.
